For Californians who work as app-based drivers for ridesharing or delivery companies and are eligible for the Prop 22 health insurance stipend, Covered California will provide a Proof of Coverage document for a given quarter once that quarter has been completed.
Covered California will mail these forms out to the address on file (so please don't forget to notify either Stride or Covered CA of any address changes), or you can find digital copies on your Covered California Account Homepage. Once you've logged into your Covered CA account, click the "View Proof of Coverage Documents" button in the "Tax Forms & Other Important Documents" section near the top of the webpage:
Don't have a Covered California account? You'll want to navigate to their "Account Creation" screen and select the "Continue" button under the "Link Existing Application" option.
- On the "Access a Current Case" screen, you'll be prompted to input a 6-digit access code to link your current case to your account. You can find this special code on your eligibility letter from Covered California or you can select the "No" response under "Do you have an Access Code to link an existing case to this new account?" and tap on the "Next" button.
- On the "Let us help you find your access code" screen, you'll be asked to input your case number, first name, last name, and date of birth. Per the instructions, "In order to link your new account to your existing case, we will need the name of the person who received the mailed letter from Covered California or a notice from your local county Medi-Cal office, and the case number listed." There's a "Get Access Code" button at the bottom you can click on if you've entered all the required information. Once you have your access code, you'll be able to fill in that field on the "Access a Current Case" page.
- Once you've completed adding user information, you'll select a username and password for your Covered California account.
- Please note: You can call Covered California at (800) 787-6921 for your case number if you don't have it, or you can simply reach out to the Member Experience team at Stride. We'd be happy to download your Proof of Coverage form and send it over to you via email.
