To update the address on your Ameritas dental or vision plan, you'll need to call Ameritas directly at (800) 300-9566.
- Once on the call select option 3
- Then option 5
Monday - Thursday 7:00 a.m. to 7:00 p.m. CT, Friday 7:00 a.m. to 5:30 p.m. CT
Note: When reaching out, you'll need to provide your name, your policy number (or DOB), and your new mailing address
