If your income qualified you for a government subsidy that made your health insurance premiums cheaper last year, you should receive a Form 1095-A in the mail. This form comes from the Marketplace (or your State's Exchange) and provides information about your insurance plan and monthly premiums last year.
- If you live in California, contact Covered CA at (800) 300-1506 to obtain a copy of your 2023 Form 1095-A (and Form FTB 3895).
- If you live in one of the following states, please refer to the State Exchange Directory for contact information: CO, CT, DC, KY, ID, ME, MD, MA, MN, NV, NJ, NM, NY, PA, RI, VT, VA, or WA.
- If you live in a Healthcare.gov state (which would be all other states not listed above), contact Healthcare.gov at (800) 318-2596.
You should also access a digital copy of your 1095-A tax form by logging into your account for Covered California, Healthcare.gov, or your State Exchange.
