To upgrade your Ameritas vision plan within 30 days of your new enrollment, please fill out and submit this form to Stride and request the change.
To upgrade your Ameritas vision plan after the initial 30 days of your enrollment you'll need to
- First, contact Ameritas at cs@ameritas to cancel your current plan
- Then, fill out and submit this form to Stride and request assistance enrolling into the new plan
Note: When reaching out to Stride, you'll need to provide your name, your policy number (or DOB), and the email address associated with your account
To move to a lower plan (a "downgrade") you'll need to have been enrolled in the plan for at least 12 months, if you have, then you should call Ameritas directly at (888) 336-7601.
- Once on the call select option 3
Monday - Thursday 8:00 a.m. to 5:00 p.m. CT, Friday 9:00 a.m. to 4:30 p.m. CT
